It is the plan in the first year of waiver approval, for Humanity in Transition, Inc. to begin providing services to in two very intimate and person-centered programs: Companion Care, Individualized Day Services, and Adult Day Supports
Companion Care
Companion services provide non-medical assistance and supervision to support a person’s goals, desires, and needs as identified in the person’s Individual Support Plan (ISP) and reflected in his or her Person-Centered Thinking and Discovery tools. Person’s supported goals may be related to the person’s safety, promotion of independence, community integration, and/or retirement.

Individualized Day Services
(IDS)
IDS is a tailored day service designed to maximize a person’s individualized choice and needs either in a 2:1 or 1:1 ratio. These tailored activities provide unique opportunities for increased integration and inclusion as well as further opportunities for the person to form valuable relationships not to mention possibly obtaining employment and volunteer experiences.

Adult Day Supports
Our facility will facilitate an array of medical, psychosocial, community-based, and person centered services. We realize that as more people decide to age in the homes opposed to institutions which are not cost effective or person centered, that it is paramount that we develop a cadre of services specific to the needs of the participants seeking ADHP services from HIT. The cadre of services include, but are not limited to nutrition counseling, exercise, health monitoring, and cognitive-stimulating activities In addition to ensuring that participants remain a vital part of their respected communities, HIT is designed to ensure that participants receive appropriate nourishment as required by their diets and intended in EPD ADHP rule.

Home & Community Based Waiver
DDS-In Home Support Provide
This is an entitlement program that will provide personal care and domestic services to aged, blind or disabled individuals in their own homes. The purpose of the program is to allow these individuals to live safely at home rather than in costly and less desirable out of home placement facilities. The range of services is determined by the Department of Disability Services, and it is the person who drives the program.
HIT will work with are groups to help provide persons the opportunity to practice social skills as well as socialize in safe environments. Individuals participate in a variety of community activities of their choice which include, but not limited to the following:
•Library-Concerts, Baseball games, Basketball games, exercise, dances, clubs, bowling
