Humanity in Transition, Inc. is minority owned 501c3 organization established in December of 2017.

The Mission of Organization is to provide people living with ID/DD/Mental illness the opportunities to maximize their highest potential by supporting them to maintain dignity, highest level of self-esteem and quality of life. 

The Organization’s specific Objectives is to support people with ID/DD to:

• Live more independently

• Improve their self esteem

• Understand their physical and emotional needs

• Develop their own individual lifestyles

• Become financially independent

• Find their voice as the most intricate part of their service delivery team

Humanity in Transition, Inc. is committed to enhancing quality of life for persons with developmental disabilities by supporting their personal choices, promoting opportunities for growth and encouraging participation as active community members by:

• Advocating for the person’s rights, choices, and self-determination;

• Assisting and supporting the person/family to direct and participate in service planning and decision making;

• Supporting informed lifestyle choices;

• Linking with natural and generic supports and services in the person’s own community;

• Promoting opportunities to develop and maintain significant relationships in their own community and;

• Coordinating and monitoring services aimed at promoting and maintaining health and while ensuring the safety and well-being of the persons.
